This San Francisco hotel is an easy walk to Fisherman’s Wharf. A crib may be added to rooms with two double beds to sleep 5, as long as one of the children is very young or an infant.
Hotel Amenities:
Free internet is available. Underground parking is very expensive. There is an on-site restaurant, and nearby cafes and restaurants of varying price ranges. There is an outdoor heated pool, which kids enjoy even if the weather is somewhat chilly. The hotel also has a small fitness center. Pets are not allowed. Up to four kids age 11 and younger eat free in the restaurant, see hotel website for guidelines.
San Francisco’s Ghirardelli Square is a few blocks away. (Don’t bother taking young kids there for fun though – it is full of pricey art and gourmet shops, though there are restaurants and sweet shops that kids might enjoy for a meal.)

Holiday Inn's own reservations line cannot book 5 people in a room. You can book the room online for 4 and then call the front desk to inform about 5 guests, the ages of the children, and request a crib. Check for price comparisons first.

I think you may have to remove Holiday Inn Fisherman’s Wharf from your list.
I contacted the front desk as suggested and was told due to fire regs I would have to get two rooms (adjoining). This was for 2 adults 3 kids (aged 8,6,4).

The front desk at Holiday Inn Fisherman’s Wharf confirmed that a roll away bed cannot be added to a room with 2 Double beds, nor to a room with a King bed with Sofa bed.
However, a crib may be added to either of those rooms. So a family of 5 with a very young child or infant could stay in one of those rooms with use of a crib.
A family should reserve either room online for four people and then add the crib request to the comment box, and also then phone the hotel’s front desk to change the number of people for the room and confirm a crib request. Be sure to indicate the ages of the children.
